Engine has been out to work on the IMS “oiler” ?

Something left out when re installing or wrong way and applying a preload to the engine and gearbox.

Engine torque applied more twist load ( torque) causing a stress crack at the weakest point ... the bell housing

That’s my theory anyway

Looking at it I think this is very likely the cause....something not correctly aligned or installed and then it is "winding up" the gearbox with engine torque. Unless it was simple mounted incorretly and too solid and vibration etc. cause the failure...I still think option 1
